Angiographic Demonstration of Multiple Bleeding Points in a Posterior Fossa Extradural Hematoma



Abstract
We report a case of posterior fossa extradural hematoma in which vertebral angiography disclosed multiple extravasations of contrast medium over the dural surface.
During the decompressive operative procedure, clotted blood, about 60 c. c. in amount, was found in posterior fossa extradural space, some of which extended suprfitentorially but much of them accumulated at the area around the confluence of sinus and the proximal part of the lateral sinuses where primary bleeding points were encountered.
Small amount of blood clot adhered to the dural surface of the posterior fossa including that of the sinuses was left unremoved as a thin layer for the prevention of further blood oozing from them.
Although multiple extravasations of contrast medium was demonstrated angiographically over the dural surface of the posterior fossa, the bleeding sources of the profuse extradural hematoma was considered as not the area in which the extravasation of contrast medium were demonstrated but the area over or around the sinuses injured. The extravasations of contrast medium was suspected to be originated from the tributaries of the meningeal branches of the vertebrobasilar artery.
